2. Name and address injector with emergency telephone number
3. Date use product (and expected duration (for example in case of Botox® 4-5 months)
4. Reconstitution / dilution information product used
5. Areas treated and units used
6. Other materials or products used
7. Allergies or sensitivities customer - or relevant history
You can even add pictures, a customer loyalty program or pre planned appointments for follow up or "touch ups".
